If you are contacting us for a billing related matter or dispute, please have your EOB (Explanation of Benefits), insurance card and other billing related information available. Our staff will be happy to assist you.

Credit card payments are accepted by many of our physicians. Please review your medical bill to find out if your physician accepts credit card payments. If your physician does accept credit cards, we will be able to assist you over the phone or through our online payment tool. Please have your Account Number (Acct No.), Group ID (GRP ID) and physician's name ready, along with your credit card information.
